community response teams (CRTs)

  • Union County developed in June 2018 with the assistance of multiple community stakeholders

  • The model shares similar values as Wraparound but does not follow the Wraparound model to fidelity.

​​

  • Teams include family/youth, their natural supports, and a core team of local professionals.

​

  • intent is to creatively problem-solve in a timely manner—streamlining referrals and getting work done in “real-time.”​

 

community response team facilitator's responsibilities:

Meeting, delegating tasks to team members, assuring collaboration among team members between meetings, and administering CANS for the youth involved as needed.

THIS IS NOT A CRISIS SERVICE

It is a responsive process
