Community Response Teams (CRTs)

 

• Union County developed in June 2018 with the assistance of multiple community stakeholders

​

• The model shares similar values as Wraparound but does not follow the Wraparound model to fidelity.

​

• Teams include family/youth, their natural supports, and a core team of local professionals.

 

• The intent is to creatively problem-solve in a timely manner—streamlining referrals and getting work done in “real-time.”

​

Community Response Team Facilitator’s Responsibilities:

Meeting, delegating tasks to team members, assuring collaboration among team members

between meetings, and administering CANS for the youth involved as needed.
